Palliative Care Hospice Prescription Examples
Bone pain in cancer does not respond well to the opioids such as Morphine, Oxycodone, Fenantyl, or Hydromorphone. However bone pain does respond well to the NSAIDs such as Ibuprofen, Naproxyn, and Ketoprofen, but these can upset the stomach. By administering the NSAID in a transdermal dosage form, bone pain is relieved at the site of pain without gastric discomfort.

Many times a patient suffers the discomfort of constipation caused by the pain medications (opioids). By adding Dextromethorpan to the dosage schedule, it is possible to reduce the dose of the opioid, thereby reducing the constipation while maintaining adequate pain relief.

Morphine, Hydromorphone and Fentanyl can be delivered by nasal spray. When administered in this nasal spray onset of relief is much faster for break through pain.

Sugar free Fentanyl or Hydromorphone Lollipops can be used for break through pain.

Transdermal Ketoprofen can be compounded in Various Strengths

Drugs that can be ordered to enhance the affect of the Transdermal NSAIDs are Bupivicaine or Lidocaine, Morphine or Hydromorphone, Gabapentin, Ketamine, Dextromethorpan.

Pilocarpine Lollipops to relieve dry mouth symptoms.

Transdermal preparations for nausea and anxiety can be prepared in a non-sticky base. The preparations can be applied and rubbed in by the loving hands of family, a healing touch. Below are several examples:

  • Promethazine in various strengths (Nausea & Vomiting))
  • Prochlorperazine (Nausea & Vomiting)
  • Ativan, Benadryl, Haldol, Reglan (ABHR) (Nausea, Anxiety, Restlessness & Pain)
  • Ativan, Benadryl, Haldol (ABH) (Anxiety Restlessness)
  • Ativan, Benadryl, Haldol, Dexamethasone (ABHD) (Nausea, Anxiety, Restlessness & Pain)
These medications can be administered by suppository. However this dosage form will results in a loss of patient dignity and increase in discomfort by having to move the patient. The patient may become dehydrated, which makes it difficult for the transdermal medications to penetrate the skin. We make a spray that can be sprayed on the transdermal medication to speed the transfer of the medication across the dry skin barrier.
